Start with the premises paper, because it decides everything else. A registered lease in the applicant's own name, or the khata extract and the latest tax receipt where the premises are owned, sits underneath every filing a Bengaluru diagnostic lab makes. The clinical establishment application names the premises. So does the establishment registration, the city corporation licence and the GST principal place of business. When the lease, the PAN and the application carry one name and one address string, the file moves. When they do not, every desk returns it separately.
Which decision comes before any application?
Settle who the applicant is before a single form opens. A pathologist can hold the lab personally, but a company or an LLP puts the licences, the lease and the equipment finance in one name that survives a partner leaving. A charitable diagnostic centre needs a trust, a society or a section 8 company, and the applicant there is the institution, never a founder personally. The choice fixes the PAN, the bank account, the registered address and the signatory on every later renewal, and changing it later means filing again.
Three Bengaluru readers, three answers:
- A pathologist opening a standalone lab in Rajajinagar, in Bengaluru West City Corporation, wanting one company name on the lease, the analyser invoices and the report letterhead.
- A Malleshwaram charitable body adding a subsidised diagnostic centre, where the applicant is the institution and its objects must already cover diagnostics.
- A Jayanagar founder in Bengaluru South selling direct to consumers online and adding home sample collection, whose collection point and processing lab are two addresses.
A coworking desk is a weak registered address for a lab. Section 12(9) of the Companies Act lets the Registrar verify a registered office physically and begin strike off where he believes no business is carried on there.
What documents and address proofs does a Bengaluru applicant need?
The pack is assembled once and then reused at every desk.
- Premises: a registered lease or licence in the applicant's name, or the khata extract with the latest property tax receipt where the premises are owned.
- People: identity and address proof for each proposed director, partner or trustee, spelled identically on the PAN, the lease and the application.
- Technical head: qualification certificates and council registration, with an appointment letter dated before the application goes in.
- Premises detail: an equipment schedule and a floor plan marking the sample collection area, the processing area and the waste holding point.
- Authority: a board resolution or an authorisation letter naming the signatory.
Two Bengaluru specific points. The e-Karmika upload takes a khata or tax receipt only as address proof, which reads as though a city corporation grants the establishment certificate. It does not. And the lease address must be the address you use everywhere, because the corporation is read off it.
Which approvals are settled, and which one do we confirm first?
The stack splits into three groups, and being honest about the third is the point.
The entity registration is settled law and runs on MCA V3. So are the Karnataka registrations that follow: establishment registration under the Karnataka Shops and Commercial Establishments Act, 1961, the professional tax enrolment and registration certificates, and the licence from your city corporation.
The health specific approvals are different. The clinical establishment registration, the biomedical waste authorisation and, where imaging is added, the radiation licence all have live departmental routing. How the registration runs, step by step
- Scope and document check. We look at the premises, the services, the headcount and the applicant, then produce the filing list for that lab.
- Entity in place. Incorporation forms go to ROC Bengaluru on MCA V3, and the company or LLP comes back with its identifier on the certificate.
- Address pack closed. The lease is finalised, the corporation is confirmed on the Greater Bengaluru Authority Know Your Ward tool, and the address string is frozen.
- Karnataka filings. Establishment registration goes through e-Karmika with the fee paid on Khajane-II, professional tax enrolment through e-Prerana at ptax.karnataka.gov.in, and the licence application to your Commissioner.
- Health approvals and handover. The clinical establishment and waste filings go in, and you receive every certificate with a renewal calendar.

The corporation licence fee is the one figure nobody should quote from memory. The Greater Bengaluru Governance Act, 2024 fixes no amount; each corporation sets its own schedule by trade and premises, so we check the live figure for your address.
Which registrar and portal actually receive the filing?
ROC Bengaluru, which the MCA also writes as ROC Bangalore, is the single registrar for the whole state of Karnataka. A lab at Rajajinagar and a lab at Whitefield file with the same office, and moving a registered office from one Bengaluru address to another does not change the registrar. Filing is on MCA V3, and there is no counter to visit.
The company comes back carrying KA as the state code inside its CIN. That matters more for a lab than for most businesses. Section 12(3)(c) of the Companies Act requires the name, the registered office address and the CIN on business letters and notices, and a lab report letterhead is exactly that. The February 2026 restructuring left ROC Bengaluru's territory alone while moving the Regional Director for Karnataka to the South-Western Region Directorate at Bengaluru.
What does a new Bengaluru lab owe in its first weeks?
Three obligations start counting from the day the lab opens, not from incorporation.
Professional tax comes first. Every non salary entry in the Karnataka Schedule sits at ₹2,500 a year, and a company registered under the Companies Act is one of them. So is anyone registered under the Karnataka Goods and Services Tax Act, 2017. Where more than one entry catches you, Explanation I means ₹2,500 is paid once and not per entry. Apply within thirty days of commencement; a late application draws ₹1,000 from an employer and ₹500 from anyone else. With staff you also take the registration certificate, deduct from any salary of ₹25,000 and above, and pay the enrolment amount before 30 April.
Establishment registration follows. The 1961 Act gives thirty days from the date the establishment commences its work. The fee runs on a headcount slab, and the certificate is valid for five years and must be displayed on the premises.
Then the municipal layer. BBMP ceased to exist on 2 September 2025 and five city corporations took its place. A licence application is addressed to the Commissioner of the corporation your premises sit in, under section 303 of the Greater Bengaluru Governance Act, 2024. Its licensing subjects include trade establishments and hospitals and nursing homes. The portal still carries the old BBMP branding at trade.bbmpgov.in. Electronic City is the exception: Phases I, II and III are an industrial township run by ELCITA, so a lab there deals with ELCITA instead.
Where do pathology lab applications usually come apart?
A lab file fails on sequence more often than on substance. The premises document gets signed in the wrong name, or the establishment registration is left until after the health filing has gone in.
